Breakdowns / Flats / Roadside Assistance

Later this year I'm going to be renting a SUV for moving. Will be using one of the major companies, likely Budget or Avis.

I've never been fully clear on what happens if you have an engine issue or flat during the rental. Do I need extra coverage for this? If so I'm guessing it's more affordable through a third party rather than the rental company.

Anyone have experience / info here?

Hi and welcome @Henry_L !

For an engine issue or a flat tire a Road Side Assistance protection is necessary. This is also valid for a dead battery, running out of fuel, lost keys or a lockout during your rental.
A lost dammage waiver is usually already included in the rental rate (minimum at Sixt).

In case you don`t rent frequently I suggest to book any extra coverage at the rental company.

If the transmission were to fail or some hose ruptured I would be responsible for repairs?

No, never.
You are also not responsible for damages due to a lack of maintenance. Therefore you pay a rental rate. ;)
